Student Disability Verification Form

$149 (Beyond ADHD Patients) | $199 (Non-Beyond ADHD Patients)

Used to determine a student's eligibility for disability-related financial aid, including grants, loans, and other support services offered by the government. Eligible students may receive up to $4,000 per school year when applying for the Canada Student Grant for Students with Permanent Disabilities (CSG-PD)

Grant for Services and Equipment Application Form

$199 (Beyond ADHD Patients) | $249 (Non-Beyond ADHD Patients)

The Canada Student Grant for Services and Equipment for Students with Permanent Disabilities (CSG-PDSE) helps cover the cost of disability-related services (for example, note takers, tutors, etc.) and equipment (ex: laptops, headphones, etc.) to help students with disabilities. Eligible Students may receive up to $20,000 per school year (reimbursed). Students must have their disability verification approved before applying for the equipment grant.

$149 (Beyond ADHD Patients) | $199 (Non-Beyond ADHD Patients)

Some Examinations and institutes/programs, may require specific Accommodation Forms for (ex: MCAT, LSAT, etc.). These forms can be completed by Beyond ADHD to help you receive the accommodations you require (ex: extra time, etc.) For current Beyond ADHD patients, academic Accommodation Letters can be provided by your Beyond ADHD NP at no additional cost.

Our Answers

Yes, all NEW patients to Beyond ADHD must book a "Step 1: ADHD Assessment" to allow your Nurse Practitioner to learn about you and your ADHD and develop a personalized treatment plan.

Before your appointment, you must send us documentation of your diagnosis and/or treatment using the secure contact button below.

We cannot refill ADHD medications if you still have a family doctor, nurse practitioner, or psychiatrist. However, we can help patients who have lost their prescriber.

Yes, we do! Please refer to our Student Disability Verification Services under our website's How It Works section.
